Immunization Requirements

All students registering or enrolled in TISD are required by the Texas Department of State Health Services to have a Certified Immunization Record (Dtap or Tdap, Polio, Hepatitis B, HIB, PCV, MMR, Varicella, Hepatitis A and Meningococcal) from a physician or other public health personnel.

- Enrolled in Medicaid
- Be an American Indian or an Alaskan native
- Does Not have Health Insurance
- or, Has Private Health Insurance that does not cover Immunizations
